Encouraging and establishing caring relationships is core to attaining optimal health outcomes, especially for clients with chronic conditions like hypertension.Building rapport enabled us to collaboratively construct specific, measurable, achievable, realistic, and time-bound (SMART) goals that matched his readiness to change, thus enhancing shared decision-making.
Strategies for Facilitating Difficult Conversations and Disclosure of Sensitive Information
I used parts of the model during my interview with Mr. James H by first establishing a confidential and non-judgmental setting, followed by inquiring about his perception of managing his hypertension. I validated his irritations with medication compliance while respectfully leading him towards an awareness of the health consequences of untreated hypertension.

This assessment will integrate process measures (how often technology has been used, compliance with SMART objectives) and outcome measures (control of blood pressure, decrease in missed doses).
